Transportation & Meals

PACE provides transportation for its participants to and from medical appointments and the PACE adult day health center. As needed, PACE also provides participants with meals in the day health center and in their homes through meal delivery services or meal prep support from the PACE Home Care team. The PACE dietary team offers nutritional education to participants, helps identify their nutritional needs, and supports them in achieving their individual nutritional goals.

Transportation

Medical and Day Health Center Transportation Services

Let our friendly drivers help you! Whether it be lack of assistance, the cost of gas, accessibility concerns, etc., our transportation department is here to help. With both accessible vans and buses, we provide transportation services to all of our participants for all medical appointments as well as transportation to and from our adult day health center. Never miss an appointment again with assistance from our transportation department!

Emergency Medical Transportation

In the event of a life threating emergency, we authorize the use of Emergency Medical Services for all participants.

Meal Services

Meals Delivered at Home

In order for our participants to improve or maintain their health, we recognize the importance quality nutrition and a balanced diet. In order to assist our participants in meeting their nutritional goals, our dietitian on staff works with all participants to evaluate their needs and determine best interventions. We have the ability to provide home delivered meals to our participants if warranted through the assessment process.

Meals at Our Adult Day Health Center

While in our day center, our participants are able to enjoy a hot meal prepared by our dietary aide. With a new menu each day, we provide much variety to ensure satisfaction across the board. Our dietitian works to ensure healthy meals that meet participants needs are served on a daily basis.

Nutrition Education and Counseling

Whether it be a goal to lose weight, a goal to gain weight, or a goal to help control blood sugar with diet, our dietitian is on-site Monday – Friday to work along side of our participants in helping them to meet their goals. One on one as well as group nutritional education and counseling are offered to all participants.
