Check Eligibility

How do I qualify for PACE?

Eligibility is determined by specific PACE enrollment criteria governed by the State of Michigan and the Centers for Medicare and Medicaid Services.

To be eligible for PACE enrollment, you must meet the following criteria:

  • 55 years of age or older
  • Meet the State of Michigan’s level of care requirement for nursing facility care
  • Able to live safely in the community with PACE services
  • Live in the PACE Central Michigan service area

After we determine that you are eligible for PACE services and complete your enrollment assessments, you will receive a personalized plan of care that includes the services you will receive and an enrollment agreement. Participation in PACE is voluntary and you may disenroll at any time.
