Prescription Drugs
With a pharmacist on staff Monday – Friday, providing medications to our participants is our priority. Packaged in a variety of ways to fit individual participant needs and delivered to each participants home, our goal is consistent access to medication as well as ensuring medication compliance.
Medication Management
In order to assist our participants with medication management, our team has a variety of interventions to meet individual needs. Alarm systems for medication reminders, locked medication wheels, multi-dose blister packs, and medication vials are a few ways we assist our participants in their homes with successful medication management while continuing to promote independence. Our homecare nursing staff is also able to complete home visits to ensure medication compliance, answer medication questions, and assist in medication set up if needed.
Durable Medical Equipment
In order to ensure that all participants enrolled under the care of PACE Central Michigan have the proper medical equipment, a home visit from a member of our therapy team is often warranted. Whether it be grab bars, a toilet raiser, etc., if we can provide something to make our participants lives easier and increase their safety, it is our priority to do so. All durable medical equipment that is recommended by the PACE team is covered 100%, with no out of pocket costs to our participants.
