Mission Statement

Our mission is to provide a full range of coordinated health and wellness services to qualified older adults in their homes, the PACE center, and the community so they may remain safely at home for as long as possible.

Designed to meet your needs

We frequently assess for your individual care and wellness needs. Together with you, we create a plan of care that meets your needs, acknowledges your preferences, and helps you reach your goals. Services are provided in your home, in the community, and at the PACE center.

One source for all of your care

By coordinating, and providing or facilitating all aspects of your care, PACE becomes your guide for navigating the often-overwhelming world of health care. You won’t have to worry about finding a ride to your appointments, paying medication co-pays, having access to healthy meals, or affording medical supplies because PACE is here to help you with all of that and so much more.

Services do require prior authorization so remember to talk to your PACE team before seeking non-emergency care or services. Care and services received without authorization could become your financially responsibility (except in an emergency), so please remember to call PACE first.
