Adult Day Health Center

The Adult Day Health Center is key to helping participants improve their health and increase their physical and emotional well-being. It is a vibrant space where PACE participants can enjoy meals, engage in activities, and socialize. While at the Center, participants can also receive personal care (as needed), participate in prescribed treatments or therapies, and access support for acute medical, emotional, or resource concerns.

On-Site Physician, Nursing Care, and Pharmacy

PACE Central Michigan clinical staff is proud to serve our participants and their medical needs. Our team is made up of our medical director, nurse practitioners, registered nurses, medical assistants, and a pharmacy technician who are on-site Monday – Friday to provide medical assistance, perform assessments, and offer medical guidance all while serving as an individual’s primary care provider. On-call services are also available to provide care and offer medical support outside of normal business hours.

Physical and Occupational Therapy and Exercise

Housed in our adult day health center is our therapy gym and therapy team. Our therapy team works with our participants to gain and/or maintain their strength and mobility to ensure safety and independence in their own home. Each participant has an individualized treatment plan that best fits their current needs to help meet their therapy goals.

Recreational Therapy and Activities

Open Monday – Friday, our adult day health center provides our participants with a variety of activities planned by our Recreational Therapist to help meet your social, emotional, cognitive, and physical needs. While here, our participants enjoy a hot meal, friendly smiles, and a day filled with structured entertainment. Structured entertainment opportunities include crafts, exercise, cognitive stimulation, and other socialization opportunities.

Meals and Nutritional Counseling

All participants have the opportunity to work with our Registered Dietitian to meet their individual nutritional or dietary goals. This includes meal planning, diabetic education, a heart healthy diet, weight management, and more. Through interventions such as home delivered meals, dietary supplements, food logs, blood sugar monitoring, and the like, PACE provides the support you need to live a healthier life.

Social Services

Our social work team works alongside the rest of the PACE team to ensure social and emotional support needs are met for all of our participants. Whether it be counseling services, assistance with Durable Power of Attorney Paperwork, safety concerns, community resources referrals, or family dynamics, our social work team is here to support you. Providing visits in the home, while in the center, or reaching out via phone, our social work team works hard to ensure all participants have the resources they need to continue to live safely in the community.
