PACE Adult Day Health Center is a vibrant community of adults who receive care and socialization together. Our facility is focused on safety, independence, and flourishing of our participants to improve and maintain their health in all aspects.
ExploreLet us assist you with your transportation and nutritional needs. We recognize the importance of providing our participants with transportation for medical appointments as well as access to well-balanced meals.
ExploreOur program includes a broad range of care and supports delivered by a team of health care professionals with a plan designed for you. Your PACE team includes experts in improving the health and well-being of older adults. They understand the obstacles you face and will guide you through exploring the opportunities you have for a better life. The team works with you to identify your goals and to help you access resources and make informed decisions that will help you reach those goals. Transportation and meal services are free to those who qualify and have these services included in their care plan.
ExploreAt PACE, we understand the needs and services necessary to improve and maintain your health, so that you may remain living independently in the community. In addition to other PACE services, consistent access to prescribed medications and medical equipment is key to helping you live a safer, healthier life at home. Once you become a PACE participant, your medications and medical equipment will be delivered to your door, free of charge.
