Our focus is on you

Our program includes a broad range of care and support services delivered by a team of health care professionals who are guided by a plan designed with you. Your PACE team includes experts in improving the health and well-being of older adults. They understand the obstacles you face and will guide you through exploring the opportunities you have for a better life. The team works with you to identify your goals and to help you access resources and make informed decisions that will help you reach those goals. Transportation and meal services are free to those who qualify and have these services included in their care plan.

Our goal is to provide older adults support they need to achieve their highest quality of life and to extend their independence for as long as possible. PACE elder care experts offer an alternative to nursing home placement that features comprehensive, coordinated care for the medical, social, and physical needs of qualified seniors. PACE also offers caregiver relief and peace of mind for families.

Your PACE Care Team

Center manager

Works to ensure that all participants needs are met by staff.

Primary care provider

Oversees your entire health care plan. On call 24 hours per day, seven days per week.

Nurses

Work closely with you and your provider to provide the medical care you need.

Home care coordinators

Coordinate the supportive services in your home.

Physical and occupational therapists

Help you stay active, meeting personal physical goals.

Dietitian

Makes sure your meals and dietary needs are met.

Social workers

Assist with everything from finding community resources to emotional support.

Recreational therapists

Provides you with opportunities for fun activities, group socialization, and individual pursuits.

Pharmacist

Makes sure all of your medication works together to ensure optimal health. Pharmacy available on-site at our Adult Day Health Center.

Transportation manager

Schedules your transportation to our day center and medical transportation.

Medical Specialists

Women’s services, dentistry and dentures, vision care, optometry, eye glasses, audiology and hearing aids, podiatry and foot care, diabetic shoes and orthotics, cardiology, rheumatology

Outpatient Services

Lab and blood tests, radiology, x-rays, and outpatient surgery

Inpatient Services

Emergency room visits, hospitalizations, inpatient specialists, skilled inpatient rehab

At Home Care

Skilled home health services such as wound care, patient and caregiver education, intravenous or nutrition therapy, injections, monitoring serious illness and unstable health status, nursing, personal care, and hospice.

At Home Services

Homemaking services such as help with chores and delivery of meals at home to help you stay independent.
