Responsibilities Will Include but not limited to:
Has the responsibility for providing direct and indirect nursing care to assigned participants. Works under the direct and indirect supervision of the Center Manager or designee but is independent in the application of advanced nursing knowledge and skills. Manages complex clinical situations for which he/she is responsible.
- Assesses the home care needs and home safety of a frail elderly population. Contributes to development of specific plans of care. Initial and periodic assessments to be conducted minimally every 6 months, complying with PACE regulations. Assist in determining Level of Care Determination for potential and current participants.
- Assist in coordination of 24-hour care delivery. Assist in coordination and implementation of homecare services including nursing, PT, OT, ST and personal care services to ensure that the quality services meet participant needs.
- Must interact efficiently and respectfully with other members/disciplines of the interdisciplinary team.
- May need to assist in conducting competencies of PCAs upon orientation.
- Demonstrates active participation in QI processes.
- Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning. Communicates any changes in participant condition to team members
- Develops a plan of care based on the nursing process which incorporates the plans of other disciplines and continuing care needs; includes the participant / family in developing or revising the plan of care; makes referrals to interdisciplinary support services. Communicates participant changes to team members
- Provides correct treatments/procedures and other care as prescribed and according to participant care standards; provides participant and family education in coordination of 24-hour care delivery; identifies emergency situations and independently initiates advanced therapy per protocol.
- Records participant care delivered as planned and any variation, with appropriate rationale; makes and records observations related to impending or associated problems; implements nursing measures related to impending or associated problems.
Education and Experience:
- RN with current MI licensure.
- At least 1 year experience as RN, preferably in either home or community health.
- Current, valid MI driver’s license and vehicle registration and proof of insurance.
- CPR certification or willingness to obtain upon hire.
- Must have at least 1 year experience in working with the frail, elderly population.
