What is a PACE program?
The Program of All-Inclusive Care for the Elderly (PACE) is a unique health care model designed to help seniors (participants) stay safely in their home for as long as possible. PACE coordinates and provides or facilitates all medical care for its participants. Services are delivered across various settings, including on site at the PACE center, in the community, and in participant homes.

Position Summary
The primary function of this position is the professional delivery of information about PACE Central Michigan to the market to generate new referrals from the community to both existing and new referral sources, then to assist in converting those referrals to enrollments into the PACE program. This position involves daily interaction and coordination of communication with prospective participants and their families, caregivers, medical professionals, the legal community, providers of senior services, and other community groups or organizations with a goal to achieve and maintain optimum census. These charges are required to occur in a manner that maintains accountability and is consistent with the mission, vision, and values of PACE Central Michigan.
What You’ll Do
- Understands, is knowledgeable, and able to professionally articulate products, services and benefits offered by PACE Central Michigan.
- Establishes and develops rapport and relationships with discharge planners, hospital staff, healthcare professionals, aging services organizations, churches, and other community members and organizations as assigned.
- Facilitates enrollment into the PACE program, including but not limited to conducting intake visits at applicant homes, gathering documentation, supporting the Medicaid application process, and scheduling in home and onsite assessments with all necessary team members.
- Prepares transition summaries and provides them to the IDT for each new enrollee.
- Prepares Enrollment Agreement, presents it to the applicant for review and signature.
- Develops and maintains a contact list for professional and community referral sources, both active and potential, and of community presentation events/opportunities, both active and potential.
- Makes informational calls to potential referral sources and closes the call w/ a follow up plan.
- Follow action steps of the Marketing Plan.
- Attends relevant team meetings at PACE Central Michigan.
- Becomes familiar with the marketplace including news and trends in the health care arena as it affects PACE.
- Become an active member in community associations relevant to our Participants.
- Tracks referral and enrollment data, prepares quarterly marketing report and provides to the Executive Director.
- Facilitates enrollment and disenrollment from PACE Central Michigan.
- Facilitates Medicaid application and redetermination process with Michigan Department of Human Services.
- Must be able to use personal vehicle for commuting to and from PACE Central Michigan as well as within our large service area.
- This position is Monday – Friday 8 am – 4:30 pm
What We’re Looking For
- Registered Nurse
- Experience in senior care admissions and/or marketing is preferred.
- Excellent assessment and communication skills with seniors age 55 and older.
