PACE Central Michigan is seeking a Resource Specialist that will work directly with the social work team, providing resources and assistance to participants of the program. The Resource Specialist will provide supportive services to program participants by facilitating access to community resources, assisting with community resource applications, working with designated advocates and the PACE interdisciplinary team to promote independence in the community.
The Program of All-Inclusive Care for the Elderly (PACE) is a unique health care model designed to help seniors (participants) stay safely in their home for as long as possible. PACE coordinates and provides or facilitates all medical care for its participants. Services are delivered across various settings, including on site at the PACE center, in the community, and in participant homes.
Responsibilities Will Include but not limited to:
- Provides general clerical support to the social work team including but not limited to making phone calls and answering department phones, preparing paperwork, reviewing medical records, and scheduling care conferences.
- Assists in assessing participant resource needs by conducting in person visits in participant homes, at facilities where participants reside, or in the PACE center.
- Refers participants and families to appropriate community agencies or facilities, acts as liaison with such organizations and as an advocate for participants.
- Facilitates Medicaid application and redetermination processes with Michigan Department of Health and Human Services.
- Documents participant changes and resource efforts and effectuation appropriately in the medical record.
Education and Experience:
- High School Diploma or equivalent.
- Associates Degree in Secretarial Science, Liberal Arts, or Health/Human Services preferred.
- Must have prior work experience in office administration, human services, or related field.
- Minimum of 1 year experience working with the frail elderly population.
- Valid MI Drivers License and proof of vehicle registration and proof of insurance.
- CPR certification or willingness to obtain upon hire.
