PACE Central Michigan is preparing for growth with our new Houghton Lake expansion and continued service in Mt. Pleasant. We are seeking a Per Diem Home Care Coordinator, RN to join our interdisciplinary team.
As our census grows, as well as the Houghton Lake expansion, there is potential for this role to evolve into full-time employment. The role provides flexibility, with shifts scheduled on an as-needed basis, and the chance to be part of a mission-driven program that helps seniors live safely at home and in their community.

What is a PACE program?
The Program of All-Inclusive Care for the Elderly (PACE) is a unique health care model designed to help seniors (participants) stay safely in their home for as long as possible. PACE coordinates and provides or facilitates all medical care for its participants. Services are delivered across various settings, including on site at the PACE center, in the community, and in participant homes.

Essential Duties and Responsibilities include but not limited to:
- Assesses, using the nursing process, the home care needs of a frail elderly population, and identifies and develops specific plans of care. Initial and periodic assessments to be conducted minimally every 6 months and must be completed prior to the scheduled team meeting. Communicates participant changes with team members.
- Coordination of 24 hour care delivery and the implementation of all home care services which include nursing, PT, OT, ST and personal care services to ensure that quality services are provided to meet participant needs.
- Coordinates and authorizes all Durable Medical Equipment.
- Coordinates and supervises all nursing care for participants in the PACE center/boarding home according to program standards. Includes interaction with other facility staff.
- Performs evaluations of HC Aides. Human resource management of HC staff in conjunction with Center Manager.
- Demonstrates active participation in QI processes
- Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning. Communicates any changes in participant condition to team members.
Qualifications:
- Minimum 1 year experience, preferably in either home or community health
- Valid State of Michigan Registered Nurse License
- Valid Michigan driver’s license and vehicle
- CPR certification.
Candidates must be available to work weekends as part of this Per Diem (PRN) position.
