Responsibilities Will Include:
The Admissions Coordinator is the initial contact for discharge planners, families, and or responsible parties for potential long term care or skilled nursing admissions. The Admissions Coordinator is also responsible for facilitating the screening process for potential long-term care or skilled nursing admissions.
· Identifies and maintains positive relationships with referral sources.
· Responsible for establishing and maintaining open effective communication between facility and referring public, consumers, physicians, and third-party payers.
· Receives, research, screens, and evaluates potential residents for appropriate clinical admission for long term / skilled services.
· Able to manage electronic referrals.
· Conducts a comprehensive medical review to determine medical necessity, appropriate source of payment and overall appropriateness of admission.
· Ensures initial and ongoing fiscal appropriateness of resident admission in collaboration with the Finance Department.
· Determines/verifies resident Medicare, Medicaid, Managed Care, and third-party insurance coverage for potential and newly admitted residents.
· Provides information to the Director of Nursing (DON) for approval regarding the resident’s medical eligibility, qualifying diagnosis, and Medicare/Managed Care eligibility based on long term /skilled care services.
· Assists in the strategizing of bed management within the facility in collaboration with the DON and the interdisciplinary team as appropriate.
Requirements:
· Associate degree or higher in the medical field.
· Working knowledge of medical terminology required.
· Knowledge of long term / skilled nursing care admissions preferred.
· Knowledge of state and federal regulations related to long term / skilled nursing care preferred.
· Knowledge of Medicare, Medicaid, Managed Care, Veteran’s Administration, and other third-party insurance coverages preferred.
· Knowledge of long term / skilled nursing facility operations preferred.
· Must have excellent computer skills to include Microsoft Office Suite and the ability to learn new applications.
· Must have excellent written and verbal communication skills.
